Emailing MAC

I am emailing MAC about a counter issue, but I am confused. Am I suppose to my address in the address box or the stores address?

Hmm... is the form specific to an issue related to a store/counter?

It's the one that says "Service at a MAC counter or Store"

Hmm... well then I would assume that it would be the address of the counter or store. I don't see what customer service would want with your address. Maybe in the commentary box add a disclaimer that you weren't sure which of the two it was referring to?
